Hi Igor!

OK, I can accept that apology. Understand, however, that I don't nowadays 
*do* (what you don't like), but I have done it and it does work. As other 
posts by others and myself have pointed out, given the time and resources 
it is of course better to take your time and tailor the treatment to each 
individual patient's presentation.

Once again, I have to emphatically credit Dr. Miriam Lee for her creativity 
in responding to the situation that she found in those early days, and the 
contribution that she has made to our profession.
