Can someone please describe the CP/M file system, it has disappeared from my 
memory.

That may have been the model.

Yes, many strange decisions in the file system.  It would be an
interesting story to know how some of them came to be.  I think
nothing could be done to improve it once Radio Shack had released,
since any competing standard, even directly from RS, would have meant
non trivial costs to the users and many compatibility issues.
