You might be interested in the story planning section of the Perplex City
retrospective. It includes flowcharts of specific parts of the game - these
were vital for planning and running live events - but also has documents
relating to characters and the long-term story.

We used OmniGraffle for charts and Google Calendar for scheduling.

When planning anything, you need to think in terms of micro and macro.
Trying to do micro planning on a macro scale can lead to insanity and a
blizzard of sticky notes. Have a broad plan that everyone understands.
Schedule the key events. Then start filling in the gaps.
