!LDRAW was dropped in versions 5 and 6 and the end-of-header marker in
version 6 [keep up there man! :-) ]
The CMDLINE is (mainly) used to indicate preferred colour of part; never
been a fan of it myself, not sure if MLCad uses it as the colour to use in
the visual parts lists (drawing black detail on a yellow base for minifig
heads, for example)
I agree that the HISTORY line shouldn't have a continuation, but list more
changes in a second line

If we are going to prefix all meta commands with LDRAW,
then LDRAW_ORG suddenly sticks out.
I suggest LDRAW_ORG is changed to LDRAW TYPE or LDRAW FILETYPE

The HELP is very generic, perhaps a second keyword would make it easier
to extend in the future.
E.g. SHORT, LONG, HTML
What is "LDraw run-time command(s)" ?
Is the CMDLINE relevant in the header ?
Long lines in HISTORY should be treated like in KEYWORDS and HELP, e.g.
0 !LDRAW_ORG HISTORY YYYY-MM-DD [UserName] Free text description of
change
0 !LDRAW_ORG HISTORY YYYY-MM-DD [UserName] Second row simulating
formatting
I also find the END-OF-HEADER unnecessary.
The last !LDRAW_ORG is the end of header.
(another indication that 0 // is a bad continuation of HISTORY :-)
