Alex,
One of the things I would try is making sure all of the socketed chips are
firmly pressed down in their sockets.
If you are still getting just the Extended Basic prompt then next would be
change the jumper settings to use flash bank 1 of the flash chip.
Bank 1 has standard Disk Basic in it or at least it should.
Then see if you can at least get to that.
If that works then bank 0 got corrupted some how. If Bank 1 still provides
only Extended Basic prompt then it is possible the flash chip went bad.
Now if you did get the stand Disk Basic prompt after switching to bank 1.
Then you can use the update disk image to try to reflash bank 0 with
SDC-DOS.

Though you will need to edit the config file on the SD card to make sure
that the update disk is mounted as drive 0 by default so you can access it.
Now if the chip went bad then you might need a replacement. Currently I
only know two people selling CoCoSDC's and that is Zippster and
retrorewind.ca which you might be able to contact them and order a
pre-programmed flash chip.
