It appears to me that some, but not all, sectors are formatting correctly,
so that when the verify pass of DSKINI is attempted, somewhere it hits a
sector that cannot be read and you get the IO error.  You might try running
a program that reads each sector in turn to see what tracks and sectors
give rise to errors.  Maybe it is something that alignment adjustment might
correct, if all errors are occurring at, say, near the hub or near the
outer edge of the disk.
