Well, I can tell you it looks like the device descriptors s0, s1, and dd all look like they built correctly as I examined them with dump and ran dmode on them, and if disk 1 boots, which it did for me, then the rest of the system probably built correctly, so chances are that the ktlr driver built correctly as well. Congratulations! Now I hope your hard drive works and you can get it configured. By the way, I think the correct values for sct and t0s are 22 (22 hex=34 decimal=17*2 sectors per track), since your controller is setup for 256 byte sectors. Make sure you run dmode on s0 before you try to format.
