I noticed you said floppy "image". I am guessing that you are formatting either a floppy image on an emulator such as VCC, or an image mounted on a DriveWire server. I will outline the procedure for these cases below. To format a real 80 track double sided floppy, you would use slightly different options so that that controller would do a low level track and sector format. That is not required or desirable on an emulator or DriveWire. These instructions are also written for NitrOS9. Microware OS-9 is slightly different.
At the prompt to format /d2 as an 80 track double sided image, do this:
dmode /d2 sid=2 cyl=50 sct=18 t0s=18 typ=20 dns=2
format /d2 R L 2 '80'
