[Coco] MPI Locking Plate Order Form

The only CoCo 2 I have had square feet and it doesn't fit.  When I fill orders this next week, I have so
Done who is testing it on a CoCo 2 that does have round feet so will let you know then.

Right now, CoCo 3 with small MPI.  It fits better than the original I bought at the Rainbowfest back in the day.
