[Coco] decode documentation

In 1989 I started working on unravelling Basic09 I-Code. In 1990 I released
the first version of DCom, which stands for DeCOMpiler. I stopped work on
DCom in 1993, as I had little choice at the time, and came back to it in
2009. At that time I decided DCom wasn't the best name for the program, so
I renamed it unpack (Basic09 packs I-Code, so my utility unpacks it).

As I worked on it, I found I needed to better understand the mechanics
behind I-Code, so I started working on the "engine" of unpack and called it
decode. Now that decode is finally at the right stage of development, I am
now beginning to work on putting the decode engine into unpack. The
difference between them is, unpack handles merged-module files, where
decode only works on one module at a time.
