if the disk won't reformat under dos/windows after you use diskini, I'd 
suspect your floppy drive is physically destroying these disks. Could be 
dirt or a damaged head scraping the media or something similar. Or, the 
disks could be old and the oxide is flaking off, which could clog up the 
heads.
