It usually happens when hitting "reply" or "reply all" and not pruning the
duplicate addresses in the "to" field.

We used to have a Yahoo+CoCo+Gmane system working, but something broke and
we stopped getting Yahoo. Here's a flow chart of sorts.

It used to be:
Yahoo <-> Maltedmedia <-> Gmane (all duplicates suppressed)

Now it is:
Yahoo -> Maltedmedia <-> Gmane (duplicates suppressed in the latter two)
