[Coco] VEF specification

On a doc I have by Bruce Isted and Tom Napolitano, VEF is called "Coco 
Vdg Enhanced Format". That suggests it is the raw screen data plus 
palette info.

The info posted by Willard is consistent with this just in case there 
are multiple formats using the VEF name.
