[arg_discuss] Post Mortem Examples?

Rogerwilco. It's not arg specific, but useful nonetheless - it was adopted
on difficult projects in my old dept at the BBC, and worked really well.

Will try and find a few moments in next week to add the glossary and upload
it somewhere useful.
