[arg_discuss] Post Mortem Examples?

I have a format document for 'after action reviews' I pulled together ages
ago, if that would help? Good for debriefing teams who work on stuff,  I'd
need to write up a bit on how to run the meeting it was designed to document
tho...

Shout if it would be useful.
